Create Surveys with MS Forms and Collect Data for Analysis Online

You can use MS Forms to create an online survey and distribute it via a shared link or QR code.

Types of questions in MS Forms:

Multiple Choice - radio buttons
Multiple Answers - checklists
Free Text Answers (short one-line or multiple-line answers)
Rating questions
Ranking questions
Likert Scales (list of questions with a rating scale for the answers)
File upload option
Date Input option
Net Promoter Score (asks respondents to rate the likelihood of recommending something)

It is also possible to apply branching rules in your forms to control the question paths in your questionnaire. Branching rules ensure that relevant questions are shown to respondents.:

Show or hide questions and add a rule to display questions based on required responses
Navigate to another question or section
